The purpose of this form is to provide information on who can assume responsibility of the student when getting home or off the bus. By dating and electronically signing this form, the parent is acknowledging and agreeing to all statements on the form.

We know that communication with parents is extremely important in our partnership to educate students. It is vital that custodial parents and guardians review and update contact information for each of their students on a regular basis.  This information is crucial not only for emergency contact purposes but also for regular communication that is sent from the district, school, teacher or extracurricular activity facilitator.  An online form entitled “Contact Update Form” is available through your Focus Parent Portal as a convenient and efficient method for you to review and update this information throughout the year.  

The purpose of this form is to allow parents to opt in or out of health screenings, administration of first aid, emergency health care and other health related items as well as disclose any new health conditions, medications, or procedures for the student. By dating and electronically signing this form, the parent is confirming complete and accurate information. 

• Technology access has been established for educational purposes and alignment with the district's curriculum and the State of Florida Standards. Students in grades 8-12 will receive a device while middle schools vary in their plan for grade level distribution. See your individual school for more information. A "Student Device Parent Acknowledgement" form is requested for each student receiving a device. • Device Insurance Information
  • All families with students enrolled in k-12 students will receive an online insurance premium invoice for $30 shortly before the start of the school year. The e-invoice is located inside of the Parent Portal Purchase & Pay tab. To view & pay invoice, a parent/guardian must log into PARENT PORTAL.To view & pay invoice, a parent/guardian must log into PARENT PORTAL.
  • Insurance coverage is optional. Families wishing to receive this coverage will have 30 days from the start of the school year to pay the invoice online with credit card. Any cash/check payments must be made on site at schools. While there is no obligation to opt for insurance coverage, the cost for device repairs is significantly reduced with a paid insurance premium for each student.
  • Students eligible for income-based services will be automatically covered and the annual premium will be waived (an invoice will appear with $0 balance in Parent Portal).  Families not auto-enrolled but wishing to claim eligibility for income-based services must submit an online form to the Office of Student Services and receive a verification letter/email from this office to resend to student's school bookkeeper. The purpose of this form is to address the requirements of Every Student Succeeds Act: Title IX/Part A. The form will need to be completed and submitted for each student. Even if the student is not homeless, the parent must fill out all required fields, select the “Not Homeless” options and submit the form. By dating and electronically signing this form, the parent is confirming complete and accurate information. 

The purpose of this form is to Provide parents with the ability to update permissions relating to the sharing of  student Information and photo, video or audio. By dating and electronically signing this form, the parents are providing their updated selections for the current school year.

Available for George Marks Elementary students only

The purpose of this form is to provide parents with a list of activities and potential risks for a student that interacts with the animals on the George Marks Elementary campus. By dating and electronically signing this form, the parent is providing permission for their child to interact with the animals. If the parent does not want to provide permission, they should not complete and submit the form; no other action is needed.

The purpose of this form is to provide parents with the ability to request an alternate first name from the student’s legal birth name as a preferred first name for their student. By dating and electronically signing this form, the parent is providing a request for schools to utilize the preferred first name. 

The purpose of this form is to allow parents to decline or allow VCS to bill Medicaid on their behalf for students provided to their student. By dating and electronically signing this form, the parent is confirming complete and accurate information. 

The purpose of this form is to identify military families and students that attend VCS. By dating and electronically signing this form, the parent is confirming complete and accurate information. 

The purpose of this form is to provide parents the opportunity to opt-out their student from Parent Resiliency Education, Civic And Character Education, And Life Skills Education teachings. By dating and electronically signing this form, the parent is confirming they do not want their child acquiring this education.

This form is for KG-8th grade students only

The purpose of this form is to provide parents information about the requirements and expectations for their student if they choose to not participate in a physical education course within their school. By dating and electronically signing this form, the parent is acknowledging and agreeing to the requirements.   

The purpose of this form is to provide parents with the ability to specify whether their student should have full or limited access to the school or classroom library.  By dating and electronically signing this form, the parent is providing the library access permissions for their student. By default, all students have access to age/grade appropriate materials in the school and classroom library.
