ECG and Physicals Requirements
Band and Color Guard (Eurythmics) students in Volusia County Schools are required to complete a health physical and an ECG (electrocardiogram) prior to participation to ensure they are medically cleared for the physical demands of rehearsals and performances. The health physical provides a comprehensive evaluation of a student’s overall health and readiness, while the ECG is used to detect potential underlying heart conditions that may not be identified during a standard exam. These requirements reflect the district’s commitment to student safety and align with procedures used for athletic programs. All documentation (3 documents linked below) must be completed and submitted to the child’s high school band director prior to participation.

a. Fill out the top portion, as well as SECTION A and attach the student’s ECG clearance report. (if Who We Play For completed your screening)
b. Fill out the top portion, as well as SECTION B if the student is being screened by a private physician.
